4. Assessment and Quiz Disclaimers

The Vital Signs Quiz, Business Vitals Assessment™, and any results, scores, or recommendations generated by this Site are for informational and educational purposes only.

• Results do not constitute professional financial, legal, accounting, or business advice.
• Assessment outcomes are based on self-reported information and generalized scoring models. They are intended as a starting point for conversation, not a definitive diagnosis of your business.
• You should not make significant business decisions based solely on assessment results without consulting with qualified professionals.
• Lynn M. Matson is not liable for any actions you take or fail to take based on quiz or assessment results.

5. Consulting Services

Consulting engagements (including diagnostics, operational blueprints, and implementation services) are governed by separate service agreements between Lynn M. Matson and the client. These Terms apply to the use of this Site, not to the terms of any consulting engagement.

If there is a conflict between these Terms and a separate consulting service agreement, the consulting service agreement shall prevail with respect to the consulting services.

6. User-Submitted Information

When you submit information through forms on the Site (contact form, diagnostic booking, quiz lead capture, or assessment responses), you represent that the information is accurate and that you have the authority to provide it. You grant us permission to use this information for the purposes described in our Privacy Policy.

We reserve the right to use anonymized, aggregated data derived from assessments and quizzes for research, service improvement, and marketing purposes, provided no individual can be identified from such data.
